Anishinabek Health Care Navigator

This program aims to support Manitoulin Island District Indigenous elders, aged 55 years and over, to remain living at home as long as possible, improving their quality of life.

Services are client-centered and promote culturally relevant health  care. These services include: discharge planning from hospital (Manitoulin, Espanola, and Sudbury), coordination of services to ensure smooth transitions and seamless care and strategies to reduce preventable (re)admissions to hospital, advocacy, and the provision of supportive resources to patients, families, caregivers, and health care partners.

The Anishinabek Health Care Navigator works in collaboration with clients, families and other community-based Traditional, health and social services providers, including Noojmowin Teg Health Centre’s Cultural Support Workers and Geriatric Social Worker, to meet the wholistic needs of clients and their supports.
