Nogdawen Dissun Diabetes Wellness Team

DWT pictureThe goal of the program is to help Manitoulin District First Nations communities and their memberships, both on and off reserve, to prevent diabetes and to manage pre- diabetes or diabetes and to lessen the burden of these conditions and risk of complications. The program does this by supportive counselling, education and foot care, and working collaboratively with other members of clients’ health care teams.

 

The Diabetes Wellness Team welcomes self-referrals as well as referrals from health care professionals. The team sees clients for individual appointments, family appointments and also engage in activities at the community level.

 

The Diabetes Wellness Team consists of:

 

  • Diabetes Wellness Dietitian, RD, CDE

  • Diabetes Wellness Foot Care Nurse

  • Diabetes Wellness Nurse, RN, CDE
