Diabetes Wellness Team (Certified Diabetes Education)
The Noojmowin Teg Health Centre (NTHC) Diabetes Wellness Team (DWT) offers a culturally appropriate approach to
diabetes self-management. Services are available to those living both on and off reserve within the Manitoulin District.
Services may be offered in person (clinic/home), virtually or phone based.

- Initial diabetes education
- Glucose monitoring (glucometer/Freestyle Libre/Dexcom)
- Medication / Anatomy overview
- Retinopathy screening
- Neuropathy screening
- Injection starts
- Insulin starts
- Intensive insulin management
- Hypoglycemia management
The DWT is a partner in shared care with Primary Care Providers (Nurse Practitioner and/or Physician).
Additionally, the DWT is able to provide referrals to external partners.
NTHC is happy to accept referrals from both healthcare providers and self-referrals.

Community Nutrition (Registered Dietitian)
The Noojmowin Teg Health Centre (NTHC) Community Nutrition offers a culturally appropriate approach to nutrition.
Services are available to those living both on and off reserve within the Manitoulin District. Services may be offered in person (clinic/home), virtually or phone based.

- Prenatal Nutrition
- Nutrition during breastfeeding
- Infant/Toddler/Child/Adolescent nutrition
- Family Nutrition
- Nutrition in the management of blood pressure, blood fats, obesity, fatty liver
- Diabetes prevention and pre-diabetes management
- Altered GI Function (gastroparesis, IBS, crohns/colitis, etc.)
- Nutrition with elders
- Disordered eating behaviors and food relationships

Advanced Foot Care
Noojmowin Teg Health Centre (NTHC) Advanced Foot Care offers culturally appropriate foot care services related to nail and skin.
Services are available to those living both on and off reserve within the Manitoulin District. Services may be offered in person or in home (dependent on individual needs)
Services available
- Nail and skin foot care
- Fungal or thickened nail care
- Callus reduction
- Athletes foot/plantar warts etc.
- Chiropody/ orthotic referrals
- Off loading device fittings and application
The Foot Care is a member of the DWT and works in shared care with Primary Care Providers (Nurse Practitioner and/or Physician). Additionally, the DWT foot care nurse is able to provide referrals to external partners. NTHC is happy to accept referrals from both healthcare providers and self-referrals.
